The objective of this study is to develop a comprehensive Dental Electronic Health Record System that integrates appointment management, patient profiling, visit tracking, and secure digital record-keeping. The system is designed to address the challenges associated with traditional manual record handling by creating a centralized digital platform where patient information, treatment histories, and reasons for visits are systematically documented. By maintaining secure digital copies, the system minimizes the risk of data loss, facilitates faster retrieval of records, and improves the overall efficiency of clinic operations. The study employed the descriptive method of research. Data were gathered through survey questionnaires administered via Google Forms after allowing the respondents to test the developed platform. The respondents included patients and individuals knowledgeable and experienced in managing dental health records and appointment processes. Their feedback was used to assess the system's functionality, usability, and effectiveness based on ISO 9126 Standards for Evaluation of Software Quality (specifically metrics on Functionality, Reliability, Usability, and Efficiency).
